Child Care Program
2012-2013

Philosophy: St. Damian's Child Care program is designed to provide a
   safe environment with activities for school-age children before and after
school

Morning Care Hours:   
Monday - Friday     7:00 - 8:00am
  $6.00 per hour or $3.00 per half hour

Afterschool Care Hours:
Monday - Friday   3:00pm  - 5:30pm
  $6.00 per hour or $3.00 per half hour

Program Plan and Schedule for Morning Care:
Children in the Morning care  may choose from a variety of games and use of the computer. A Child care provider supervises the children during indoor play.

Program Plan and Schedule for Afternoon Care:
After school the children walk from their classrooms to the after school room. The child care providers check in each child. Children are given time to eat their nutritional snacks and change into their play clothes. Child care providers plan and supervise children participating in many activities. These activities include outdoor play, homework and free choice of materials.

Our After School Program offers a playground with a slide, swings, and a playscape along with other recreational playground equipment.

St. Damian Child Care Parents:

 

 The following forms must be completed and returned to the school office no later than Friday, August 17th.  We must have all forms on file before your child's starting date in the child care program.

 

_____      Child Care Registration Form and Deposit

                $30.00 one child or $50.00 per family

_____      Emergency Card – complete both sides and fill in ALL boxes/lines. Nothing blank
                 ****Fill out an Emergency card for each child.****

_____      Child Care Program Guideline sheet - signed

_____      Health Appraisal Form (Physical Form) current and signed by a doctor
                (this form may be obtained  from your doctor or the school office)
                       
                                                              OR
 
                 a signed statement from the parent stating that:
                (a) your child is in good health with activity restrictions noted.
                (b) your child’s immunizations are up-to-date
                (c) the immunization record or appropriate waiver is on file with the child’s school.
